Sports Physicals

If you have an entering 6th - 12th grader that needs a sports physical, the Menomonie Athletic Booster Club and Prevea Health will be offering sports physicals at the Menomonie High School Fieldhouse on Friday, July 28th from 8am - 12pm.
No appointment necessary. First-come, first-served.
The physical form must be completed and signed by a parent. Save time and complete it before you arrive. Physicals will not be performed without all pages of the form completed and signed.
Physical Form
Please note: Physicals are good for two years.
There is no charge for the physical, however a suggested donation of $20 to support the booster club is appreciated. Cash and checks only please.
The sports meeting at the High School on August 2nd is for High School students only.
Information and sign ups for Middle School Athletics will be available at the Meet and Greet Day on Monday, August 28th in the Middle School cafeteria.
